LIFE Program key points |

| • Using the LIFE Website, you can log and track your own healthy activities. | ||
| • The LIFE Website also gives you access to many educational tools. | ||
| • Enrollment in LIFE requires completing a Health Risk Appraisal on-line. | ||
| • The LIFE website is open to any MCCH Employee. | ||
| • Employees self-report LIFE activities. | ||
| • Employees on the MCCH Health Insurance Plan can earn higher discounts. | ||

LIFE Website Instructions |
||
| In order to participate in the LIFE program you must enroll on the LIFE Website and create a personal account. | ||
| 1.) First, go to the LIFE Website, https://mcch.motivation.cc/. | ||
| 2.) Select Create One Now and follow the instructions. | ||
| 3.) For Enrollment Code type in: MCCH | ||
| 4.) In the Population box select: MCCH Employee | ||
| 5.) In the Group select: LIFE. | ||
| 6.) For the Program select: Other. | ||
| 7.) Your User Name should be your 5 digit employee number. | ||
| 8.) Your User Password is a unique
password that you create. It must contain 8 characters and include one capital letter and one non-letter. Example Password: G8trfanS |

| You will be required to complete the Health Risk Appraisal to complete enrollment. | ||
| All of your responses will be kept
confidential. No one at MCCH has access to your specific HRA responses. |

Claiming your LIFE Points |
||
| Once you have enrolled on the LIFE Website, you can begin claiming LIFE points. | ||
| Each LIFE Accrual period is 6 months. | ||
| The periods run from: January 1 -- June 30 and July 1 -- December 31 each year. | ||
| The points earned in a 6 month period will qualify you for discounts on your MCCH Health Insurance for the following 6 month period. | ||
| After you logon to the LIFE Website, click on Earn Wellness Points on the left-hand side of the motivation homepage. | ||
| Use the pull down calendar at the top of the page to indicate the date for each activity that you claim. | ||
| To get more information about each LIFE item listed and to ensure that you meet the criteria to claim it, click the blue question mark ( ? ) next to the item. | ||
